Corioamnionitits: un reto diagnóstico

Resumen

Introducción: La corioamnionitis se define como la inflamación de las membranas ovulares corio y amnios en respuesta a la invasión microbiana u otro proceso patológico; puede ser clínica cuando las manifestaciones clínicas de inflamación local y sistémica están presentes, subclínica cuando no presenta signos clínicos o histológica ante la evidencia microscópica de la infección o la inflamación en examen de la placenta. La frecuencia de corioamnionitis es muy variable; depende de los factores de riesgo de la población estudiada, los criterios usados para su diagnóstico (clínico o histológico) y la edad gestacional. Esta entidad clínica es causa de importantes secuelas maternas, fetales y neonatales como endometritis posparto, absceso pélvico y alto riesgo de cesárea en la madre; prematuridad, muerte perinatal, neumonía, sepsis, hemorragia intraventricular y parálisis cerebral en el neonato. Realizar un diagnóstico precoz y adecuado es fundamental para realizar intervenciones orientadas a prevenir dichas secuelas. Objetivo: Hacer una revisión bibliográfica sobre el diagnóstico de corioamnionitis. Metodología: Se realizó una búsqueda de la literatura publicada durante los últimos 5 años sobre el diagnóstico de corioamnionitis en las bases de datos Pubmed,  tripdatabase, Science direct y Embase biomedical. Conclusiones: A pesar de los avances de la medicina en métodos diagnósticos, establecer parámetros unificados para el diagnóstico precoz de corioamnionitis aún sigue siendo un reto, como lo muestra la variabilidad de la evidencia encontrada en esta revisión. Aún priman los hallazgos clínicos y la pericia del médico para tener la sospecha diagnostica y orientar su conducta a la realización de acciones tempranas encaminadas a la prevención de las secuelas maternas y fetales de la corioamnionitis.
